PPP RULES AND GUIDELINES

PingPongParkinson, Inc. (hereinafter, “PPP”):

The individual named below (referred to as "I" or "me") desires to participate in the PingPongParkinson program (the "Activity"). As consideration for the intangible value that I will gain by participating in the Activity, I agree to all the rules and guidelines set forth in this form:

1. I understand that my participation in PPP is contingent on preregistration online and prepayment for my class because I may be contacted outside of the Activity (in case of unintended consequences due to COVID)
2. Classes will strictly be limited to 60 or 90 minute sessions (depending on location)
3. I will not participate in doubles table tennis (ONLY SINGLE) because I understand premises on which the Activities take place must operate at 50% capacity to ensure social distancing (of 6 feet)
4. I understand that my temperature must be checked at the entrance of the facility, and I may be refused admission if it is greater than 100.0F
5. While PPP will provide paddles, balls and juggling sacks for participants (which will be sanitized before and after use), I acknowledge that all equipment (including the tables) may be fomites (objects which may carry virus particles)
6. If I bring my own equipment (i.e.- paddle or juggling sack), it must be labeled
7. I agree to wear a face mask at all times
8. While hand sanitizers will be provided, I will also regularly wash hands (before and after playing)
9. If hydration is required, I will bring my own LABELED water bottle
10. I will not move seating that is provided on the periphery of the facility (as it will be strategically placed to ensure appropriate distancing)

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE RULES AND GUIDELINES AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE RELEASEES.
